Have an Australian friend who – back in the day – had a job testing foreign-trained professionals for English proficiency. Over the years, though, she realized that English skills were not enough; cultural differences were there which could – and did – cause real problems. One of the main ones was with respect to risk – our culture is extremely risk-adverse, while other cultures will chance an operation as a last hope.
Some years ago, there was a trial in – I think – Winnipeg – where a pediatric surgeon was charged with incompetence following the deaths of several infants. To this day, I suspect a strong element in the whole case was very different expectations of outcomes: the surgeon was saying “this might not work, but it’s the last chance for your child”, while the parents were hearing “this will save your baby”.
To be fair, though, it’s not just foreign-trained health professionals who cause problems. Remember the heart-breaking case of an Ontario couple whose baby underwent many medical interventions which weren’t helping. Yet when the parents asked that their baby – then about nine months old – just be allowed to die, Children’s Services intervened and convinced the court there was hope. The baby was taken from the care of the parents, moved to a hospital away from the home, and finally died there, far from the arms of his parents. The parents went public, openly accusing at least one official of lying about the prognosis for their child in order to obtain guardianship. I still remember the mother’s words which were to the effect of “When they say they’re doing everything possible for child, it’s not good”.

So, they (the provinces, the College of Physicians, and the different medical schools) make it nearly impossible for Canadian-born, Canadian-educated, English-Canadians to get into med school (one guy I knew waited so many years, he got a Masters of Science on top of his Bachelor of Science…and then he was “only” accepted to Dental School. He figured that was his only shot, so he’s a dentist now instead of a doctor. The Francophone Quebecers have it a little easier because they don’t need a science degree to get into med school.
A simple solution is for the Muslim applicants for Canadian medical certification had to have a prerequisite. That of serving a year in a small animal clinic working on dogs.
Those who still had 10 fingers left at the end of the year may qualify for further training.
Those without enough fingers left to do a handshake with, they won’t have to worry about having to shake a woman’s hand.

Having worked with foreign trained soft war ware developers let me just say if your doctor struggles with English gtf out ASAP.
Foreign medical grads come in all varieties. There was a time when we knew, (and we still do), which countries produced medical trainees whose standards were roughly equivalent to our own. We would approve their qualifications and go from there. Then someone cried, “discrimination” and you can guess the rest. Some FMGs have a strong academic/theoretical knowledge of medicine, and no or minimal hands on training. One FMG I worked with was so bad I asked her, not sarcastically, “How did you get into medicine?” Answer, “I come from a good family.” Some, with training go on to become excellent physicians. Some, I wouldn’t allow to touch my cat. The biggest cultural challenges I see come in the thought process. Some have the attitude, “I’m the doctor, so you should be happy with whatever I did and accept whatever I tell you.” They’re a problem. Some want a formulaic approach to diagnosis and treatment i.e. if x then y. I try to teach a more “plastic” approach. Some get it. Some don’t. I’ve nixed a couple from our program who just didn’t get it, and encouraged a couple more who went on to be outstanding. The key is, treat each one as an individual case and don’t be afraid to pull the plug on the duds.

Funny story…when I was posted to an airbase in Greenwood, NS I had to get my physical done by one of the base doctors. Mine happened to be a woman. So later that day, it was a Friday, at the Officers’ Mess, I introduced my girlfriend to her…with the preamble…Since both of you have fondled my balls, I figured you should meet….Then hilarity ensued.

Many women say they are more comfortable with a woman Doctor (certainly ObGyn) than a male. So why would it be any less legit for me to prefer “my people” … who speak my language … to care for my medical needs ? My health doesn’t give a damn about political correctitudes !
The best way to become a doctor in Canada is to leave the country, get your MD in the USA, practice there for a while, then come back to Canada. And do another year of residency, because American doctors might not be smart enough to practice in Canada, y’know.
Otherwise, you better have a 4.0 GPA from Grade 11 onward, and be really good at kissing people’s asses. And you better be anything other than a White male.
The part I love about all this is that the type of human being it takes to get a 4.0 GPA all the way through 6 years of school is not really who you want as a practicing physician. They tend to be somewhat brittle, and they don’t care about other humans. If they did, they wouldn’t have locked themselves away studying their whole young lives.

While we have standards that eliminate 90% of the Canadian kids who want to be doctors, we encourage recruitment of butchers who graduated from 3rd world schools. It’s the same as Canadian food standards being high but we allow the import of repackaged Chinese dog vomit labelled as food.
