2. With regards to the “remodelling”, I’m reminded of this comment by Oliver Cromwell:

   Similarly, I once knew a draftsman who, when he specified tolerances on his shop drawings, took a conservative approach. The worst that could happen was that more material had to be removed from the workpiece in order to get it to fit. It’s rather hard to “un-remove” (i. e., add) steel to a shaft if it turns out that it was machined to a diameter that’s too small.

      I have the same questions, Bunny. The lack of context for the graph rings alarm bells. The inference is that the surgeries are related to trannies getting “fixed”, but there’s nothing about the graph that supports that.
