Very interesting analysis. In the comments there is some speculation about Doug Ford running, with others being dismissive. I would like to see him run. I saw him on that Political Blind Date show on TVO, and he is a charmer. I think he does not have Rob Ford`s weaknesses. Anyway, I was not planning to vote for the PC`s this time (either not vote, or — more likely — vote for the Trillium candidate in our riding). Doug Ford could bring me back into the fold — and I am not even that much of a social conservative. It is just that the party insiders seem like a bunch of jackals. Ford would be a refreshing change — just like Trump.

The failure of the o-ring to seal properly was only a symptom of what caused the mishap.
The problems started when the decision was made to use only a single manned vehicle instead of two (an orbiter and a booster). They continued with the selection of using solid rocket motors rather than liquid-fuelled engines. (The solids, once ignited, couldn’t be shut down or throttled.) There were also questions about how the contractor for the solid rocket boosters, Morton Thiokol, was selected.
The final design of the space shuttle was also questionable, such as the lack of a proper crew escape system.
In addition, the management of the shuttle program also contributed to what happened. Problems with the shuttle that were noted by crews during preceding missions were either trivialized or ignored. Astronaut John Young (who died a few days ago) was critical of how things were run and his reward from NASA was to be sidelined and, in effect, removed from flight status.
The Rogers Commission (whose members included the aforementioned Dr. Feynmann as well as former astronaut Sally Ride, former astronaut Neil Armstrong, and former test pilot Chuck Yeager) investigated the incident. It concluded that it was an “accident rooted in history”.
